Crawlspace doors are usually made from metal. Metal will rust if it is in contact with water. Doors usually get rained on or is set to close to the ground allowing water to settle next to it. The options are to replace with another metal door, (allowing the rust process to start over again) or to replace with a treated lumber hinged door. If the opening is to small for easy access a larger door can usually be created.
